Sen. Tammy Baldwin asked the VA to investigate care at the Tomah VA Medical Center after a former hospice nurse was arrested on patient-abuse charges. Baldwin also cited employee reports of staffing and care concerns at the facility.
Baldwin wrote to VA Secretary Doug Collins requesting an immediate review of staffing and care at the Tomah VA, as well as an investigation by the VA Office of Inspector General. She said employees had reported poor management, inadequate staffing and concerns about hospice care, including delays in medication and insufficient provider coverage.
The former nurse was arrested after investigators alleged she withheld oxygen from a hospice patient in August 2025. A criminal complaint says the patient was without oxygen for about an hour and a half and died six days later; the district attorney accused the nurse of trying to hasten the patient’s death. Baldwin requested an accounting of the VA’s response and steps taken to protect other patients. The facility did not immediately respond to a request for comment.
